Summary

This case study highlights pulmonary endometriosis, a rare lung condition. Findings showed cyclical lung abnormalities during menstruation, confirming catamenial hemoptysis.

Area of Science:

  • Pulmonology
  • Gynecology
  • Radiology

Background:

  • Pulmonary endometriosis is a rare condition characterized by endometrial tissue in the lungs.
  • Catamenial hemoptysis, the coughing up of blood related to the menstrual cycle, is a rare manifestation.

Observation:

  • A 25-year-old female presented with recurrent hemoptysis exclusively during her menstrual cycle.
  • Thoracic computed tomography (CT) during menstruation revealed diffuse ground-glass opacities and acinar nodules in the left lung's superior lower lobe.
  • These radiological findings were absent on CT scans taken outside the menstrual period.

Findings:

  • The cyclical nature of hemoptysis coinciding with menstruation, coupled with specific radiological findings on thoracic CT, led to the diagnosis of catamenial hemoptysis.

  • The observed lung changes were attributed to pulmonary endometriosis.
  • Implications:

    • This case underscores the importance of considering rare diagnoses like pulmonary endometriosis in patients presenting with cyclical respiratory symptoms.
    • Accurate diagnosis relies on correlating clinical presentation with imaging findings, particularly during the symptomatic phase.
    • Further research into the pathogenesis and optimal management of pulmonary endometriosis is warranted.
