- 3,4-Methylenedioxymethamphetamine (MDMA, ecstasy) is a recreational drug associated with potential long-term neurotoxic effects.
- Serotonin (5-HT) systems are primary targets of MDMA, but its impact on human brain 5-HT synthesis capacity remains incompletely understood.
Purpose of the Study:
- To compare brain serotonin (5-HT) synthesis capacity between MDMA polydrug users and healthy controls.
- To investigate gender differences in MDMA's effects on 5-HT synthesis.
- To examine the relationship between MDMA use characteristics and regional 5-HT synthesis.

- Positron emission tomography (PET) was employed using the tracer alpha-[(11) C]methyl-l-tryptophan ((11) C-AMT).
- PET scans assessed 5-HT synthesis capacity in 17 drug-free MDMA users and 18 matched healthy controls.
- Regional (11) C-AMT trapping was analyzed in relation to gender and MDMA use patterns.
Main Results:
- MDMA users showed reduced (11) C-AMT trapping in pre-frontal, orbitofrontal, and parietal regions compared to controls.
- These reductions were more pronounced in males.
- Elevated (11) C-AMT trapping was observed in the brainstem and frontal/temporal areas of MDMA users.
- (11) C-AMT trapping in the brainstem and pre-frontal regions correlated with cumulative MDMA exposure and recency of use.
Conclusions:
- Regionally decreased 5-HT synthesis in the forebrain of MDMA users suggests potential MDMA-induced neurotoxicity.
- Increased 5-HT synthesis in the brainstem may represent a compensatory response.
- Pre-existing alterations in 5-HT systems cannot be entirely excluded as a predisposing factor for MDMA use.
