Abstract:
Difficulty accessing patient information, duplication of effort, and breakdowns in communication within the medical infrastructure are some of the major issues challenging the quality of care and the efficiency of the Canadian healthcare system. In addition, patients are demanding faster access to the right services, improved outcomes, and more involvement in managing their own healthcare. This article presents a framework for patient-centred care that allows healthcare providers to deliver better and faster patient interventions with treatment plans based on a patient's complete medical history.
