    Summary

    Serum thyroxine-binding globulin (TBG) and T4 levels are elevated in active liver diseases and hepatocellular carcinoma (HCC). Normal free T4 (FT4) levels in these patients rule out hyperthyroidism.

    Context:

    • Thyroid hormone levels and binding proteins are often altered in liver disease.
    • Understanding these alterations is crucial for accurate diagnosis and management of thyroid function in patients with hepatic conditions.

    Purpose:

    • To investigate serum thyroxine-binding globulin (TBG) and thyroid hormone levels in patients with various liver diseases.
    • To differentiate thyroid dysfunction from altered binding protein levels in liver disease.

    Summary:

    • Radioimmunoassay measurements of TBG, T4, T3, and FT4 were performed in patients with HBsAg carriers, acute hepatitis, chronic hepatitis, cirrhosis, hepatocellular carcinoma (HCC), and metastatic liver cancer, compared to controls.
    • Significantly elevated TBG and T4 were observed in acute hepatitis, chronic active hepatitis, active cirrhosis, HCC, and metastatic liver cancer. T3 was elevated in chronic active hepatitis and active cirrhosis.
    • TBG levels correlated with alanine transaminase (ALT), suggesting a link to hepatocytolysis in chronic hepatitis and potential synthesis by malignant cells in HCC. Normal FT4 excluded hyperthyroidism.

    Impact:

    • Provides insights into the relationship between liver disease activity and TBG levels.
    • Helps in the differential diagnosis of thyroid abnormalities in liver disease patients.
    • Suggests TBG elevation in HCC may be due to tumor cell synthesis.
