Cirrhosis I: Introduction01:23

Cirrhosis I: Introduction

24
Cirrhosis is a chronic, irreversible liver disease characterized by the widespread replacement of healthy liver tissue with fibrotic scar tissue and the formation of regenerative nodules.Etiology of cirrhosisCirrhosis results from sustained liver injury that triggers progressive fibrosis and structural remodeling. The underlying causes are diverse, encompassing common and less frequent clinical conditions. Regardless of the origin, all causes lead to chronic inflammation, hepatocyte loss, and...

Nephrotic Syndrome I : Introduction

1.1K
Nephrotic Syndrome is a chronic kidney disorder defined by clinical findings such as severe proteinuria, hypoalbuminemia, hyperlipidemia, and edema. These symptoms result from damage to the glomeruli, the kidney’s filtering units, increasing their permeability to proteins.Definition and Meaning:Proteinuria, defined as the loss of more than 3.5 grams of protein per day in adults, is a crucial feature of nephrotic syndrome. This condition is often accompanied by edema, the accumulation of...

Amyloid Fibrils

10.1K
Amyloid fibrils are aggregates of misfolded proteins.  Under most circumstances, misfolded proteins are either refolded by chaperone proteins or degraded by the proteasome. However, in the case of a mutation or a disease, these proteins can accumulate to form large clusters and often further assemble to form elongated fibers, called fibrils.
Amyloid deposits were observed as early as 1639 in the liver and the spleen.   In 1854, Rudolph Virchow performed iodine staining,...

Area of Science:

  • Urology
  • Nephrology
  • Hematology

Background:

  • Penile amyloidosis is typically a manifestation of primary cutaneous amyloidosis.
  • Systemic amyloidosis rarely presents with penile involvement as an initial symptom.

Observation:

  • Two patients presented with a penile glans ulcer as their first clinical manifestation.
  • Immunohistochemical analysis confirmed lambda light chain type amyloid deposits.

Findings:

  • The penile ulcer was the initial sign leading to the diagnosis of systemic amyloidosis.
  • Both patients exhibited nail dystrophy, including onycholysis, trachyonychia, and onychoschizia.

Implications:

  • This case series highlights a previously unreported initial presentation of systemic amyloidosis.
  • Early recognition of penile ulcers as a potential sign of systemic amyloidosis is crucial for timely diagnosis and management.
