Background:

  • Phrenic nerve nodules are typically primitive tumors, with surgery serving as both diagnostic and therapeutic. This case presents an unusual etiology for diaphragmatic relaxation.

Observation:

  • A Caucasian male presented with shortness of breath and fatigue, exhibiting right diaphragmatic palsy on chest X-ray. Imaging revealed a nodular lesion on the right phrenic nerve.

Findings:

  • Positron emission tomography was negative for glucose uptake, leading to a preoperative diagnosis of a neurogenic tumor. Histopathological examination confirmed an idiopathic inflammatory nodule of the phrenic nerve.

Implications:

  • This case introduces a previously unreported cause of diaphragmatic relaxation. Surgical resection of the inflammatory nodule was both diagnostic and therapeutic, resolving the patient's symptoms.
