Background:

Splenic hydatid cysts are rare, often asymptomatic, and challenging to diagnose preoperatively.
Echinococcus granulosus infection can manifest in various organs, including the spleen, posing diagnostic dilemmas.

Observation:

A 27-year-old female presented with left hypochondrium pain and mild splenomegaly.
Abdominal ultrasonography and contrast-enhanced CT revealed cystic lesions in the spleen, suggestive of hydatid cysts.
Serological testing confirmed IgG antibodies against Echinococcus.

Findings:

Histopathological examination of the resected spleen confirmed an infected hydatid cyst with necrosis and Echinococcus hooklets.
The patient underwent splenectomy after pneumococcal vaccination, with no intraoperative or postoperative complications.

Implications:

Accurate preoperative diagnosis of splenic hydatid cysts is crucial to implement necessary precautions during surgery.
Preventing cyst rupture during excision is vital to avoid anaphylactic shock and surgical field contamination.
Increased clinical awareness of splenic hydatid disease is essential for timely diagnosis and management.
