Summary

Bioactive surfaces enable long-term extracorporeal circulation (ECC) without systemic heparinization. This study demonstrates the feasibility of heparin-free ECC for extended lung support using these advanced materials.

Background:

  • Heparin is traditionally required for extracorporeal circulation (ECC) to prevent clotting.
  • Bioactive surfaces (HBS) have shown promise in reducing the need for anticoagulation during ECC.

Purpose of the Study:

  • To investigate the possibility of completely avoiding systemic heparinization during long-term ECC using HBS.

Main Methods:

  • Seven conscious sheep underwent venovenous bypass with HBS-coated equipment for 24 hours (n=3) or 5 days (n=4).
  • No heparin was administered throughout the procedures.
  • Standardized conditions, including blood flow rates and priming volume, were maintained.

  • Coagulation parameters, including partial thromboplastin time and activated clotting time, remained within physiological ranges.
  • Platelet counts stayed above 80% of baseline, and hemolysis rates were negligible.

  • Conclusions:

    • Bioactive surfaces (HBS) can be safely used for long-term extracorporeal circulation.
    • Complete avoidance of systemic heparinization is achievable with HBS in long-term ECC.
    • This finding has significant implications for reducing bleeding risks associated with anticoagulation in ECC.
