Summary

Susceptibility-weighted imaging effectively detects intracranial hemorrhage in heat stroke patients. This imaging technique may also help predict patient outcomes, identifying those at higher risk of severe complications or death.

Background:

  • Heat stroke is a critical medical emergency characterized by severe hyperthermia.
  • Complications include disseminated intravascular coagulation and potential neurological damage.

Purpose of the Study:

  • To evaluate the utility of susceptibility-weighted imaging (SWI) in identifying intracranial hemorrhage (ICH) post-heat stroke.
  • To determine if SWI can aid in predicting prognosis for heat stroke patients.

Main Methods:

  • Eight heat stroke patients with Glasgow Coma Scale scores of 3-9 underwent MRI.
  • Susceptibility-weighted imaging was performed 2-5 days after heat stroke onset using a 1.5 T scanner.

Main Results:

  • SWI detected punctate hemorrhages in the brainstem, corona radiata, and frontal lobe in three patients.
  • Two of these patients died within 25 days; the third entered a persistent vegetative state and died later.
  • The five patients without detected hemorrhages showed gradual recovery with residual cerebellar dysfunction.

Conclusions:

  • Heat stroke can lead to life-threatening intracranial hemorrhages.
  • Susceptibility-weighted imaging is a valuable tool for detecting these hemorrhages.
  • SWI shows potential for prognostication in heat stroke patients.
