Background:

  • Pre-operative decolonization therapy (DcTx) using chlorhexidine (CHG) and mupirocin can decrease surgical site infections (SSIs).
  • Patient compliance with traditional DcTx is often suboptimal, limiting its effectiveness.

Purpose of the Study:

  • To evaluate the efficacy of an immediate DcTx combining intranasal antimicrobial photodisinfection therapy (PDT) with CHG body wipes.
  • To assess the impact of this novel approach on reducing SSIs.

Main Methods:

  • A prospective study involving 3068 patients undergoing various elective surgeries from September 2011 to August 2012.
  • Patients received CHG wipes pre-operatively and intranasal PDT immediately before surgery.
  • Results were compared to a historical control group of 12,387 patients and a concurrent control group of 206 untreated patients.

Main Results:

  • The treated group showed a significant reduction in SSI rates compared to the historical control group (1.6% vs. 2.7%, P = 0.0004).
  • High compliance (94%) was achieved with the new DcTx protocol.
  • Propensity score analysis confirmed that DcTx significantly reduced SSI risk (P = 0.00026).

Conclusions:

  • The combination of CHG wipes and PDT is an effective strategy for reducing SSIs.
  • This immediate pre-operative DcTx achieved excellent compliance and was readily integrated into surgical workflows.
