    Background:

    • Peripheral giant cell granuloma (PGCG) is a common reactive lesion of the oral cavity, typically presenting on the gingiva or alveolar ridge.
    • The etiology of PGCG remains unknown, but it is considered an inflammatory hyperplastic response to local irritants.
    • Histologically, PGCG is characterized by numerous multinucleated giant cells in a background of mononuclear stromal cells and hemosiderin deposition.

    Observation:

    • A 75-year-old male presented with a painless, purplish-red, sessile soft tissue nodule in the posterior mandibular region.
    • Radiographic examination, including orthopantomogram and cone beam computed tomography, revealed a well-demarcated radiolucency with evidence of mandibular bone resorption.
    • The clinical and radiographic findings suggested a soft tissue lesion with osseous involvement.

    Findings:

    • Histopathological examination of the excisional biopsy confirmed the diagnosis of Peripheral Giant Cell Granuloma (PGCG).
    • The lesion exhibited characteristic features of multinucleated giant cells, mononuclear stromal cells, and extravasated red blood cells.
    • Multimodality diagnostic approach (clinical, radiographic, histopathologic) was crucial for accurate PGCG diagnosis.

    Implications:

    • This case highlights the importance of a comprehensive diagnostic workup for mandibular swellings, even when seemingly benign.
    • Early and accurate diagnosis of PGCG is essential for appropriate management and to prevent unnecessary aggressive treatments.
    • Understanding the clinical and radiographic presentation of PGCG aids in differentiating it from other oral pathologies.
