Summary

Severe gingival recession, especially Miller Class IV, poses challenges for root coverage. A novel pedicled buccal fat pad (PBFP) technique shows promise for predictable outcomes in challenging cases.

Background:

Gingival recession is multifactorial, with limited success in root coverage for advanced cases (Miller Class III and IV).
Current techniques often yield suboptimal results for severe gingival recession defects.

Observation:

The pedicled buccal fat pad (PBFP) is a well-vascularized tissue with stable volume, suitable for reconstructive purposes.
PBFP offers a readily available local flap for increasing keratinized mucosa and aiding root coverage.

Findings:

This case report details a new technique utilizing PBFP for root coverage in a challenging Miller Class IV gingival recession.
The pedicled buccal fat pad (PBFP) technique was applied to a Miller Class IV recession on a maxillary right first molar.

Implications:

The pedicled buccal fat pad (PBFP) technique presents a potentially predictable solution for severe gingival recession, particularly Miller Class IV.
This approach may improve treatment outcomes and increase keratinized tissue dimensions in challenging periodontal reconstructions.
