Summary

Trabecular titanium (TT) acetabular implants show reliable biological fixation in hip revision surgery. These implants offer excellent early results, though longer follow-up is needed for long-term assessment.

Area of Science:

  • Orthopedic surgery
  • Biomaterials science
  • Implant technology

Background:

  • Acetabular revision surgery addresses complex bone defects after total hip replacement.
  • Trabecular titanium (TT) implants offer porous structures for bone ingrowth.
  • Evaluating early outcomes of novel implant designs is crucial for clinical adoption.

Purpose of the Study:

  • To assess the early clinical outcomes of cementless acetabular revisions using trabecular titanium (TT) modular implants.
  • To evaluate the efficacy and safety of TT implants in various acetabular defect classifications.

Main Methods:

  • Retrospective analysis of 81 acetabular revisions using TT implants (March 2009-May 2012).
  • Classification of acetabular defects using the Paprosky system.
  • Clinical assessment via modified Merle d'Aubigné-Postel score with a mean follow-up of 38.14 months.

Main Results:

  • Significant improvement in functional scores from 4.7 pre-operatively to 9.8 post-operatively.
  • One case of acetabular component instability successfully managed with a cage system.
  • Cranial migration observed in three Paprosky type 3B defects, which stabilized without further intervention.
  • No dislocations or modular component dissociations; one fatigue fracture of a hemispherical module reported.

Conclusions:

  • Trabecular titanium (TT) acetabular cups, modules, and augments provide reliable biological fixation in revision hip surgery.
  • The implants demonstrate excellent early functional and radiographic results.
  • Long-term performance evaluation of TT modular implants is warranted.
