Summary

Delayed sciatic nerve repair in rabbits showed no negative effects on nerve regeneration. Immediate or delayed neurorrhaphy yielded similar outcomes in axon count, nerve structure, and mechanical properties after 8 weeks.

Background:

  • Peripheral nerve injuries necessitate prompt clinical treatment.
  • However, delayed surgical repair (neurorrhaphy) is sometimes unavoidable.
  • The impact of delayed repair on nerve regeneration outcomes remains a clinical concern.

Purpose of the Study:

  • To evaluate the effects of immediate versus delayed sciatic nerve repair on nerve regeneration.
  • To compare histological and biomechanical outcomes following different repair timings.

Main Methods:

  • Transected sciatic nerves in New Zealand rabbits were repaired either immediately or after a 3-day delay.
  • Stereological, histomorphological, and biomechanical analyses were performed at 8 weeks post-surgery.
  • Evaluated parameters included axon number/area, myelin/epineurium thickness, Schwann cell morphology, and nerve fiber mechanical properties.

Main Results:

  • No significant differences were observed in axon number and area between immediate and delayed repair groups.
  • Myelin sheath and epineurium thickness, as well as Schwann cell morphology, were comparable across both groups.
  • Biomechanical properties of the regenerated nerve fibers did not differ significantly between the immediate and delayed neurorrhaphy groups.

Conclusions:

  • Delayed neurorrhaphy (3 days post-injury) does not appear to have deleterious effects on sciatic nerve repair.
  • These findings suggest that a delay in surgical intervention for sciatic nerve transection may be clinically feasible without compromising regeneration outcomes.
  • Further research could explore longer delay intervals and different nerve injury models.
