Summary

For cesarean section (CS) or breech extraction of second twins, breech extraction under local anesthesia is safer. This method is recommended over CS for rapid twin delivery when both options are available.

Area of Science:

  • Obstetrics and Gynecology
  • Perinatal Medicine
  • Neonatal Outcomes

Background:

  • Second twins often require rapid delivery interventions.
  • Cesarean section (CS) and breech extraction are common methods for second twin delivery.
  • Maternal anesthesia type can impact neonatal outcomes.

Purpose of the Study:

  • To compare outcomes of second twins delivered via CS versus breech extraction.
  • To evaluate the impact of maternal anesthesia on neonatal Apgar scores.
  • To determine the safest delivery method for second twins requiring rapid intervention.

Main Methods:

  • Retrospective cohort study of second twins in Sweden (1973-1981).
  • Comparison of second twins delivered by CS after vaginal first twin birth with breech-extracted second twins.

Related Experiment Videos

  • Analysis of medical records for maternal morbidity and neonatal Apgar scores.
  • Main Results:

    • Second twins born under maternal general anesthesia had lower 5-minute Apgar scores.
    • Cesarean section group experienced substantial maternal morbidity.
    • Breech extraction under local anesthesia showed favorable outcomes compared to CS.

    Conclusions:

    • Breech extraction under local anesthesia is preferable to CS for rapid second twin delivery.
    • Local anesthesia for breech extraction minimizes risks associated with general anesthesia.
    • Clinical guidelines should favor breech extraction when rapid second twin delivery is necessary and both methods are feasible.
