Aneurysm III: Interprofessional Care

Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. Hybrid aortic arch repair offers a less invasive alternative to conventional surgery for complex aortic dissections. This approach demonstrated a 100% stent graft deployment and patency rate with no strokes in a selected patient group.

Area of Science:

  • Cardiovascular Surgery
  • Vascular Surgery
  • Thoracic Surgery

Background:

  • Conventional open surgery for thoracic aortic dissections carries significant mortality and morbidity risks.
  • Complex aortic arch dissections present unique surgical challenges.

Purpose of the Study:

  • To analyze the experience with hybrid aortic arch repair for complex aortic arch dissections.
  • To evaluate the safety and efficacy of this less invasive approach.

Main Methods:

  • Retrospective analysis of 18 patients undergoing hybrid aortic arch repair between 2009 and 2013.
  • Individualized procedural strategies including trifurcation/bifurcation grafts, ascending aorta replacement, and extra-anatomic bypass grafting.
  • 100% stent graft deployment rate achieved.

Main Results:

  • No strokes or early to midterm endoleaks were observed.
  • One patient expired due to heart failure three months post-procedure.
  • Median follow-up of 20 months showed a 100% graft patency rate.

Conclusions:

  • Hybrid aortic arch repair provides a safe and effective treatment option for selected patients with complex aortic pathologies.
  • Various debranching techniques allow for tailored, less invasive solutions compared to open surgery.
