Area of Science:

  • Psychiatry
  • Psychometrics
  • Digital Health

Background:

  • The internet is increasingly utilized for suicide research and prevention.
  • Optimizing online assessment requires brief, high-quality tools for suicidal behavior risk.
  • Computer adaptive testing (CAT) can enhance accuracy and reduce patient burden in online assessments.

Purpose of the Study:

  • To evaluate if a computer adaptive simulation can shorten the Beck Scale for Suicide Ideation (BSS).

Main Methods:

  • Utilized data from the PITSTOP suicide study.
  • Applied principal components analysis (PCA), confirmatory factor analysis (CFA), and a graded response model (GRM).
  • Simulated a computer adaptive test (CAT) for the BSS.

Main Results:

  • Analyzed data from 505 patients.
  • The BSS demonstrated unidimensionality and good internal consistency.
  • CAT simulation indicated 4 items suffice for estimating suicidal behavior risk, on average, reducing the original 19 items.

Conclusions:

  • CAT successfully reduced the length of the Dutch BSS.
  • CAT application can enhance accuracy and decrease response burden for online suicide risk assessment.
  • A practical example of the CAT simulation for the Dutch BSS is provided.
