Summary

Computer adaptive testing (CAT) significantly shortened the Beck Scale for Suicide Ideation (BSS) from 19 to 4 items for online suicide risk assessment. This improves accuracy and reduces patient burden.

Background:

The internet is increasingly utilized for suicide research and prevention.
Optimizing online assessment requires brief, high-quality tools for suicidal behavior risk.
Computer adaptive testing (CAT) can enhance accuracy and reduce patient burden in online assessments.

Purpose of the Study:

To evaluate if a computer adaptive simulation can shorten the Beck Scale for Suicide Ideation (BSS).

Main Methods:

Utilized data from the PITSTOP suicide study.
Applied principal components analysis (PCA), confirmatory factor analysis (CFA), and a graded response model (GRM).
Simulated a computer adaptive test (CAT) for the BSS.

Main Results:

Analyzed data from 505 patients.
The BSS demonstrated unidimensionality and good internal consistency.
CAT simulation indicated 4 items suffice for estimating suicidal behavior risk, on average, reducing the original 19 items.

Conclusions:

CAT successfully reduced the length of the Dutch BSS.
CAT application can enhance accuracy and decrease response burden for online suicide risk assessment.
A practical example of the CAT simulation for the Dutch BSS is provided.
