Background:

  • Penicillin G (PNG) is known to induce T-cell responses in vitro in allergic patients.
  • The precise mechanisms and forms of PNG determinants responsible for cellular proliferation remained unclear.

Purpose of the Study:

  • To investigate the immune response to different PNG preparations in patients with proven PNG allergy.
  • To elucidate the role of PNG determinants in eliciting T-cell mediated proliferation.

Main Methods:

  • Isolation of peripheral blood lymphocytes (PBL) from patients with proven PNG allergy.
  • Evaluation of proliferative responsiveness to soluble PNG, PNG-protein conjugates (BPO-HSA, BPO-PL, BPO-HEX), and non-reactive penicilloate salts.
  • Utilized autologous mixed lymphocyte culture (MLC) with penicilloylated stimulator cells to assess responsiveness to membrane-bound PNG.

Main Results:

  • Potentially reactive PNG (1000 µg/ml) and penicilloylated autologous cells significantly stimulated PBL proliferation.
  • Non-reactive PNG salts and soluble PNG conjugates did not elicit a stimulatory response.
  • PNG-modified autologous cells were identified as potent stimulators in PNG-allergic individuals.

Conclusions:

  • T-cell immunity in PNG-allergic patients is directed towards PNG-modified 'self' antigens.
  • This finding clarifies the form of PNG determinants responsible for cellular proliferation in penicillin allergy.
