Acute kidney injury (AKI) after coronary artery bypass grafting (CABG) significantly increases the long-term risk of end-stage renal disease (ESRD). Even mild AKI nearly triples ESRD risk, highlighting the importance of kidney function monitoring post-CABG.

Background:
- Acute kidney injury (AKI) is a frequent complication following coronary artery bypass grafting (CABG).
- AKI post-CABG is linked to adverse outcomes, but its long-term impact on end-stage renal disease (ESRD) risk remains unclear.
Purpose of the Study:
- To investigate the association between AKI after CABG and the subsequent risk of developing ESRD.
- To quantify the long-term renal risk associated with varying stages of AKI post-CABG.
Main Methods:
- A nationwide cohort study in Sweden (2000-2008) included 29,330 patients undergoing primary isolated CABG.
- AKI was staged using the Acute Kidney Injury Network (AKIN) classification based on serum creatinine changes.
- Cox proportional hazards regression models were employed to assess ESRD risk.
Main Results:
- Postoperative AKI occurred in 13% of patients.
- Over a mean follow-up of 4.3 years, 0.4% of patients developed ESRD.
- Multivariable analysis revealed a 2.92-fold increased hazard for ESRD in AKIN stage 1 and a 3.81-fold increased hazard in AKIN stage 2-3, compared to no AKI.
Conclusions:
- A minor increase in postoperative serum creatinine levels post-CABG is associated with a substantially elevated long-term risk of ESRD.
- These findings underscore the importance of renal function monitoring and management in patients following CABG, even with mild AKI.
