Summary

Lung transplant graft shortage is a challenge for small adult patients. Combining graft size reduction with ex vivo lung perfusion successfully improved lung function in a patient, offering a new solution.

  • A novel approach combined lobar transplantation with contralateral nonanatomic graft reduction during ex vivo lung perfusion.
  • This technique was applied to a lung initially deemed unsuitable for transplantation.

  • Surgical manipulation of the lung graft during ex vivo lung perfusion did not impair functional improvement.
  • The patient demonstrated functional recovery comparable to standard bilateral lung transplantation.

Implications:

  • This combined technique offers a viable solution to overcome lung graft shortages.
  • It expands the potential donor pool by enabling the use of oversized or previously unsuitable grafts.
  • This approach may improve transplant opportunities for small adult patients awaiting lung transplants.
