Background:

  • Multiple sclerosis (MS) is a neurodegenerative disease often associated with cognitive impairments.
  • Autobiographical memory (AM) deficits have been anecdotally reported in MS patients, but their functional basis remains unclear.

Purpose of the Study:

  • To investigate the functional brain mechanisms underlying autobiographical memory (AM) impairment in multiple sclerosis (MS).
  • To compare brain activation patterns during AM tasks between MS patients and healthy controls.

Main Methods:

  • 18 MS patients and 18 healthy controls completed the autobiographical interview (AI).
  • Functional magnetic resonance imaging (fMRI) assessed brain activity during AM construction and elaboration tasks.
  • A categorical control task was used for comparison.

Main Results:

  • MS patients scored significantly lower on the AI compared to controls (p < .001).
  • Despite lower AI scores, MS patients performed efficiently on the fMRI AM task.
  • Patients exhibited increased bilateral cerebral activations in prefrontal, temporal, and posterior regions during AM tasks (p < .005).
  • Right hippocampal recruitment was notably higher in MS patients, correlating with memory access difficulty and detail (p < .005).

Conclusions:

  • Findings suggest functional cerebral reorganization and compensatory mechanisms in MS patients during AM tasks.
  • The study supports the presence of an autobiographical memory retrieval deficit in multiple sclerosis.
  • Altered neural activity in core AM networks, particularly involving the prefrontal cortex and hippocampus, is implicated in MS-related memory impairment.
