Purpose of the Study:

  • To examine the association between surgical case volume and hospital management efficiency.
  • To determine if increased experience at hospital and surgeon levels impacts postoperative outcomes and length of stay.

Main Methods:

  • Analysis of 909 patients undergoing surgery for unruptured abdominal aortic aneurysms (2006-2008).
  • Multilevel logistic regression to assess the relationship between case volume (hospital and surgeon level) and postoperative complications.
  • Multilevel log-linear regression to analyze the association between case volume and length of stay (LOS) pre- and post-surgery.

Main Results:

  • Higher annual surgical case volume at both hospital and surgeon levels was significantly associated with reduced odds of postoperative complications (OR 0.981 and 0.982, respectively).
  • Increased hospital and surgeon case volume correlated with shorter postoperative length of stay.
  • Higher hospital-level case volume was linked to a reduction in preoperative length of stay, but surgeon-level volume was not.

Conclusions:

  • Hospital-level experience appears to enhance hospital management efficiency.
  • Increased surgical volume contributes to better patient outcomes and shorter lengths of stay.
