Background:

  • Primary dural lymphoma is an exceedingly rare subtype of primary central nervous system (CNS) lymphoma.
  • It originates from the dura mater without evidence of systemic disease elsewhere in the body.

Observation:

  • A 47-year-old woman presented with an asymptomatic, indolent mass in the right frontal region.
  • Magnetic resonance imaging (MRI) confirmed the mass lesion, with no focal neurological deficits noted on examination.

Findings:

  • Surgical resection via right frontal craniectomy was performed.
  • Histological examination revealed diffuse large B-cell lymphoma (DLBCL).
  • The patient received six cycles of the R-CHOP chemotherapy regimen (rituximab, cyclophosphamide, adriamycin, vincristine, prednisolone).

Implications:

  • The patient achieved complete remission with no neurological deficits and no tumor recurrence at 72 months post-surgery.
  • This case highlights the efficacy of R-CHOP chemotherapy for primary dural lymphoma.
  • Individualized, multidisciplinary treatment approaches and vigilant follow-up are crucial in managing this rare condition due to the lack of established standard protocols.
