Summary

Nontuberculous mycobacteria (NTM) were found in 21.2% of patients with diffuse panbronchiolitis (DPB), suggesting a higher prevalence in this condition. Impaired mucociliary clearance may increase susceptibility to NTM infection.

Background:

  • Nontuberculous mycobacterial (NTM) lung disease is known in cystic fibrosis (CF).
  • Limited data exists on NTM prevalence in non-CF bronchiectasis.
  • Diffuse panbronchiolitis (DPB) is characterized by reduced mucociliary clearance and bronchiectasis.

Purpose of the Study:

  • To investigate the prevalence of NTM in patients with diffuse panbronchiolitis (DPB).
  • To explore the association between NTM infection and DPB characteristics.

Main Methods:

  • Retrospective review of mycobacterial cultures, patient characteristics, and CT findings.
  • Study included 33 patients diagnosed with DPB between January 2000 and December 2012.
  • Prevalence determined by at least one positive NTM culture from sputum.

Main Results:

  • NTM prevalence in sputum was 21.2% (7 out of 33 patients) over a mean follow-up of 162.8 months.
  • Mycobacterium avium complex was the most common NTM species identified.
  • NTM-positive patients had significantly lower forced expiratory volume in 1s (% predicted) compared to NTM-negative patients.

Conclusions:

  • NTM infections appear to be more common in patients with DPB.
  • Impaired mucociliary clearance, a hallmark of DPB, may predispose individuals to NTM infection.
