Purpose of the Study:

  • To evaluate the predictive value of resting ECG findings for cardiac disease in healthy individuals.
  • To assess the role of resting ECG in screening for coronary artery disease.

Main Methods:

  • A comprehensive literature search of English-language studies from 1970-1988.
  • Analysis of 40 articles reporting on long-term survival of healthy individuals with and without abnormal ECG findings.
  • Calculation of relative risk for coronary artery disease and 95% confidence intervals.

Main Results:

  • Certain abnormal ECG findings (ventricular premature beats, left axis deviation, left ventricular hypertrophy, myocardial ischemia) are associated with a small increase in coronary artery disease mortality risk in healthy middle-aged men.
  • No evidence indicates that early detection of these ECG abnormalities leads to interventions that improve health outcomes.
  • Baseline ECG tracings showed minimal impact on emergency room decision-making.

Conclusions:

  • The current evidence does not support the use of screening ECGs in individuals without signs of cardiac disease or cardiovascular risk factors.
  • Routine ECG screening for coronary artery disease in asymptomatic men is not recommended based on available data.
