Summary

An intact cone outer segment tips (COST) line after epiretinal membrane (ERM) surgery correlates with better visual acuity. However, some patients achieve good vision despite a disrupted COST line 12 months post-surgery.

Purpose of the Study:

  • To investigate the association between the recovery of the cone outer segment tips (COST) line and best-corrected visual acuity (BCVA) following epiretinal membrane (ERM) removal surgery.

Main Methods:

  • A retrospective analysis of 61 eyes from 56 patients undergoing idiopathic ERM removal.
  • Spectral-domain optical coherence tomography (SD-OCT) was used to assess the integrity of the external limiting membrane, inner/outer segment (IS/OS) junction, and COST line.
  • Correlation between morphological parameters and BCVA was determined at multiple postoperative time points.

Main Results:

  • An intact COST line was significantly associated with better BCVA at 1, 6, and 12 months post-ERM surgery (p < 0.001).
  • The IS/OS line showed a stronger correlation with BCVA than the COST line or external limiting membrane via multivariate analysis.
  • Despite good BCVA (≥20/20), 30.8% of eyes had a nonintact COST line at 12 months.

Conclusions:

  • Maintaining COST line integrity after ERM surgery is linked to improved visual acuity.
  • Significant visual recovery can occur even with persistent COST line disruption 12 months post-surgery, suggesting other factors contribute to visual outcomes.
