Summary

Persistent pain after total knee arthroplasty (TKA) is linked to widespread hypersensitivity to pain and increased neuropathic pain. Patients with moderate to severe pain showed reduced pressure pain thresholds and heightened sensitivity to cold and heat.

Background:

  • Up to 15% of patients experience persistent moderate to severe pain after total knee arthroplasty (TKA).
  • This post-TKA pain may be associated with widespread hyperalgesia and neuropathic pain characteristics.

Purpose of the Study:

  • To investigate differences in pain sensitivity and neuropathic pain reporting between patients with and without moderate to severe pain at least 12 months post-TKA.
  • Specifically comparing pressure pain thresholds, thermal (cold/heat) pain and detection thresholds, and self-reported neuropathic pain.

Main Methods:

  • A comparative study involving 53 volunteers, categorized into moderate-to-severe pain or no-pain groups based on the Knee Society Score©.
  • Utilized independent t-tests to analyze differences in pressure pain, thermal thresholds, and self-reported neuropathic pain between the two groups.

Main Results:

  • The moderate-to-severe pain group demonstrated reduced pressure pain thresholds at the knee and elbow.
  • This group exhibited increased sensitivity to cold (knee and elbow) and heat (elbow), along with impaired cold and heat detection thresholds at the elbow and heat detection at the knee.
  • Significantly higher self-reported neuropathic pain was reported in the moderate-to-severe pain group.

Conclusions:

  • Persistent pain following TKA is associated with widespread hypersensitivity to pressure and cold (hyperalgesia).
  • Increased reporting of neuropathic-type pain is a key characteristic of patients experiencing persistent moderate to severe pain after TKA.
