Summary

Kit-form mercapto-acetyltriglycine (MAG-3) is suitable for routine renal imaging and semiquantitative analysis. While MAG-3 differs from ortho-iodohippurate (OIH) in renal handling, dynamic studies are comparable. Further kinetic research is needed for quantitative analysis.

Area of Science:

  • Nuclear Medicine
  • Radiopharmaceutical Science
  • Renal Physiology

Background:

  • 99mTc-labelled mercapto-acetyltriglycine (MAG-3) is a proposed substitute for ortho-iodohippurate (OIH) in dynamic renal imaging.
  • Clinical experience and biokinetic data for MAG-3 are limited.
  • Previous studies often used HPLC-purified MAG-3, which is not practical for routine use.

Purpose of the Study:

  • To evaluate the suitability of a non-HPLC-purified MAG-3 kit for routine renal imaging.
  • To compare the renal handling and dynamic imaging characteristics of MAG-3 with OIH.
  • To assess the potential of MAG-3 as an OIH substitute in clinical settings.

Main Methods:

  • A commercially available MAG-3 kit (non-HPLC) was administered to 10 normal subjects and 15 patients with renal disease.

Related Experiment Videos

  • Dynamic renal imaging was performed.
  • Semiquantitative analysis of renal uptake and excretion was conducted.
  • Comparison of MAG-3 imaging criteria with those used for OIH.
  • Main Results:

    • MAG-3, in kit form, proved suitable for renal imaging and semiquantitative analysis in a routine clinical setting.
    • Significant differences in renal handling were observed between MAG-3 and OIH.
    • Dynamic studies with MAG-3 could be interpreted using the same criteria as OIH studies.

    Conclusions:

    • Kit-form MAG-3 is appropriate for routine renal imaging and semiquantitative assessment.
    • Despite differing renal handling, MAG-3 dynamic studies are comparable to OIH studies for interpretation.
    • Additional research into MAG-3 kinetics is required for its use in quantitative studies like effective renal plasma flow determination.
