Background:

Observation:

  • A 35-year-old woman with congenital uterine absence received a uterus from a living 61-year-old donor.
  • In vitro fertilization was performed prior to transplantation, with 11 embryos cryopreserved.
  • The recipient experienced regular menstruation post-transplantation and underwent embryo transfer one year later.

Findings:

  • The first embryo transfer resulted in a successful pregnancy, maintained under triple immunosuppression.
  • The pregnancy was complicated by mild rejection episodes and pre-eclampsia, managed with corticosteroids.
  • A healthy male infant was delivered via Cesarean section at 31 weeks and 5 days gestation.

Implications:

  • This case establishes uterus transplantation as a viable option for treating uterine factor infertility.
  • Live uterus donation, including from postmenopausal donors, is demonstrated as feasible.
  • Successful pregnancy and livebirth following uterus transplantation opens new possibilities for assisted reproduction.
