  • Care coordination is crucial in primary care, yet communication and care failures occur when patients see multiple providers.

Purpose of the Study:

  • To evaluate a novel Care Coordination Model's effectiveness within the Safety Net Medical Home Initiative (SNMHI).
  • To assess if implementing the model improves care coordination in primary care practices.

Main Methods:

  • Correlation analysis was used to link the Care Coordination Model's elements (Key Activities Checklist - KAC) with care coordination outcomes (PCMH-A).
  • SNMHI practices self-assessed their implementation of 12 KAC items across four occasions.
  • The study correlated KAC items with 5 PCMH-A items measuring care coordination capability.

Main Results:

  • Elements of the Care Coordination Model showed positive correlations with some, but not all, PCMH-A care coordination measures.
  • Model-related activities were most strongly associated with improved follow-up for patients discharged from hospitals or seen in emergency departments.

Conclusions:

  • Implementing activities consistent with the Care Coordination Model provides suggestive evidence for enhancing care coordination in safety net primary care.
  • Further research is necessary to confirm the model's impact and refine its application.
