Background:

Schilder's disease, a rare demyelinating disorder, presents with varied neurological symptoms.
Early diagnosis and intervention are crucial for managing progressive neurological conditions.

Observation:

A 14-year-old female presented with acute bilateral visual loss.
Neuroimaging revealed symmetrical occipital lobe lesions and a frontal lobe lesion.
Cerebrospinal fluid analysis showed elevated immunoglobulin G and oligoclonal bands.

Findings:

Biopsy confirmed a demyelinating disorder without cytoplasmic inclusions.
The patient experienced a dramatic visual recovery and lesion shrinkage after steroid therapy.

Implications:

This case highlights the effectiveness of steroid treatment in Schilder's disease.
Prompt diagnosis and treatment can significantly improve visual function and neurological outcomes.
Further research into the immunopathogenesis of Schilder's disease is warranted.
