Summary

This study details a rare case of hand gigantism, Macrodystrophia Lipomatosa, treated with conservative surgery. Microsurgical techniques and a dermal substitute (INTEGRATM) improved both cosmetic and functional outcomes.

Area of Science:

  • Hand surgery
  • Microsurgery
  • Tissue engineering

Background:

  • Macrodystrophia Lipomatosa is a rare condition causing limb overgrowth.
  • Differential diagnosis and conservative surgical treatment are crucial for managing such rare soft tissue masses.

Observation:

  • A rare case of digital and ulnar side of the hand gigantism with massive fatty infiltration was surgically treated.
  • The procedure involved microsurgical neurolysis of the ulnar nerve and covering the neurovascular bundle with INTEGRATM, a dermal substitute.

Findings:

  • The patient experienced good cosmetic and functional results at a three-year follow-up.
  • Histological examination of the INTEGRATM interface revealed newly formed tissue rich in precursor cells.

Implications:

  • Conservative surgery, utilizing microsurgical skills and dermal templates like INTEGRATM, is effective for aggressive soft tissue masses in the upper limb.
  • This case highlights the potential of dermal substitutes in reconstructive hand surgery.
