Blastomycosis osteomyelitis, often mimicking bone tumors, can be rapidly diagnosed with intraoperative frozen section. Early consideration in endemic areas improves patient outcomes for this rare fungal infection.

Background:

  • Blastomycosis osteomyelitis is a rare complication of Blastomyces dermatitidis infection.
  • Diagnosis is often delayed due to its infrequent occurrence and potential to mimic other conditions.
  • The utility of intraoperative consultation for rapid diagnosis has not been previously detailed.

Purpose of the Study:

  • To evaluate the role of intraoperative consultation in the diagnosis of blastomycosis osteomyelitis.
  • To review clinicopathologic features of blastomycosis osteomyelitis and arthritis.
  • To assess the diagnostic accuracy of frozen section for this condition.

Main Methods:

  • Retrospective chart review of 14 cases diagnosed between 2000-2010.
  • Analysis of clinicopathologic information, including presentation, affected sites, and diagnostic methods.
  • Evaluation of frozen section accuracy and correlation with microbiologic studies.

Main Results:

  • Twelve of 14 cases presented as bone tumors, predominantly in the lower extremities near the knee.
  • Septic arthritis was commonly secondary to adjacent osteomyelitis.
