Summary
Intraosseous hemangiomas are rare benign bone tumors. Surgical excision and Medpor reconstruction effectively treated a left infraorbital margin hemangioma with no recurrence.

- These lesions primarily affect soft tissues, making intraosseous occurrence uncommon.
Observation:
- A 42-year-old woman presented with a painless, hard swelling on her left infraorbital margin.
- Orbital magnetic resonance imaging (MRI) diagnosed the swelling as an intraosseous hemangioma.
Findings:
- En bloc excision of the intraosseous hemangioma was performed via a subciliary incision.
- Reconstruction of the infraorbital margin defect utilized Medpor, secured with polypropylene sutures.
- Histopathological analysis confirmed the diagnosis of intraosseous cavernous hemangioma.
Implications:
- Total surgical excision followed by reconstruction is the recommended treatment for intraosseous hemangiomas.
- Medpor proved effective for reconstructing the orbital floor, infraorbital rim, and maxillary sinus wall, achieving a natural contour without additional scarring.
- The patient showed no signs of recurrence or cosmetic deformity three months post-surgery.
