T Cell Types and Functions01:24

T Cell Types and Functions

3.1K
When T cells with CD4 markers are activated, they give rise to two types of effector cells: helper T cells and regulatory T cells. Meanwhile, T cells with CD8 markers differentiate into effector cytotoxic T cells. The differentiation of CD4 T cells into helper T cell subsets, such as Th1, Th2, and Th17 cells, is dependent on the antigen type, antigen-presenting cell, and regulatory cytokines.
Th1 cells stimulate dendritic cells to express necessary co-stimulatory molecules on their surfaces for...

Drugs for Treatment of Crohn's Disease in IBD Using Immunomodulatory Agents01:29

Drugs for Treatment of Crohn's Disease in IBD Using Immunomodulatory Agents

734
Crohn's disease is an inflammatory bowel disorder marked by chronic inflammation of the GI tract. Various treatment strategies for Crohn's disease are employed, such as immunomodulatory agents, glucocorticoids, and biologics or anti-TNF therapy. Azathioprine (Imuran), a commonly used immunomodulatory drug for Crohn's disease, is converted in the body to mercaptopurine, which inhibits purine biosynthesis and cell proliferation. Both are utilized in severe cases of Inflammatory Bowel...

Drugs for Treatment of Crohn's Disease in IBD Using Biologic Agents: Anti-TNF

854
Tumor Necrosis Factor (TNF), a proinflammatory cytokine, contributes significantly to the inflammation seen in Crohn's disease. It exists as soluble TNF and membrane-bound TNF, with actions mediated through TNF receptors (TNFR). TNFR activation leads to the release of proinflammatory cytokines, T-cell activation, collagen production, and leukocyte migration, all contributing to inflammation in Crohn's disease. Anti-TNF monoclonal antibodies, namely infliximab (Remicade), adalimumab...

Chronic Inflammation: Introduction01:12

Chronic Inflammation: Introduction

7
Chronic inflammation is a prolonged, dysregulated immune response that persists for weeks to years when the inciting stimulus is difficult to eradicate or when self‑antigens drive ongoing reactivity. Morphologically, it is defined by mononuclear cell infiltration, progressive tissue destruction, and concurrent attempts at healing via angiogenesis and fibrosis. Compared with acute inflammation, edema is less prominent while cellular infiltration predominates; triggers include persistent...

Inflammatory Response01:28

Inflammatory Response

12.2K
An inflammatory response is a localized, nonspecific immune reaction that occurs when a tissue is injured. It is characterized by redness, swelling, heat, and pain, which are commonly called the cardinal signs and symptoms of inflammation. Inflammation can sometimes result in a loss of function.
Inflammation can be triggered by various stimuli, such as impact, abrasion, chemical irritation, infections, and extreme hot or cold temperatures. These can damage cells and connective tissue fibers,...

Drugs Used in Lower Respiratory Disorders: Overview

1.5K
Lower respiratory tract disorders present challenges that often require skilled and nuanced approaches for effective management. Common ailments, such as asthma and chronic obstructive pulmonary disease (COPD), have prompted the development of intricate treatment strategies involving bronchodilators and anti-inflammatory drugs, each tailored to ease breathing and revitalize the lungs.
Bronchodilators, the first step of respiration enhancement, come in various forms, each with its own mechanism...

Summary

Interleukin-6 (IL-6) blockade shows excellent efficacy in rheumatoid arthritis but heterogeneous results in other chronic inflammatory diseases. This review summarizes IL-6 blockade data and safety concerns for rheumatic diseases.

Area of Science:

  • Immunology
  • Rheumatology
  • Cytokine Biology

Background:

  • Proinflammatory cytokines, like Interleukin-6 (IL-6), are key in rheumatic disease development.
  • IL-6 has diverse roles including immune regulation, hematopoiesis, inflammation, and oncogenesis.
  • Targeting IL-6 is a therapeutic strategy for inflammatory conditions.

Purpose of the Study:

  • To review current data on IL-6 blockade in chronic inflammatory diseases.
  • To discuss the clinical efficacy of IL-6 inhibitors.
  • To evaluate the safety profile of IL-6 blockade therapies.

Main Methods:

  • Literature review of clinical trials and basic research on IL-6.
  • Analysis of data on IL-6 blockade efficacy in various rheumatic diseases.
  • Examination of safety data from IL-6 inhibitor therapies.

Main Results:

  • IL-6 blockade demonstrated excellent clinical efficacy in rheumatoid arthritis.
  • Therapeutic outcomes for other chronic inflammatory diseases treated with IL-6 inhibitors were varied.
  • Safety data regarding IL-6 blockade requires careful consideration.

Conclusions:

  • IL-6 is a critical target in rheumatologic diseases.
  • While effective in some conditions like rheumatoid arthritis, IL-6 blockade efficacy is inconsistent across all chronic inflammatory diseases.
  • Further research and careful patient selection are needed to optimize IL-6 blockade therapy and manage safety concerns.
