[Primary bone marrow diffuse large B cell lymphoma: three case reports and literature review]

Background:

• Diffuse large B cell lymphoma (DLBCL) is a common non-Hodgkin lymphoma.
• Primary bone marrow DLBCL, lacking nodal involvement, is exceptionally rare.
• Early recognition and accurate diagnosis are crucial for effective management.

Observation:

• Three elderly patients (56-70 years) presented with abnormal blood counts and aggressive disease.
• No enlarged lymph nodes, liver, or spleen were detected on physical or imaging examinations.
• Diagnosis was confirmed via bone marrow morphology, flow cytometry, and immunohistochemistry.

Findings:

• All three patients diagnosed with primary bone marrow DLBCL.
• Treatment with rituximab combined chemotherapy resulted in complete response.
• Longer follow-up is necessary to assess long-term survival outcomes.

Implications:

• This report is the first of primary bone marrow DLBCL in China.
• Improved recognition of this rare entity is needed for timely diagnosis.
• Further research into the pathogenesis and optimal therapeutic strategies for primary bone marrow DLBCL is warranted.
