Purpose of the Study:

  • To investigate how modifying meal composition or ingestion duration affects postprandial endothelial function.
  • To evaluate the impact of altered gastric emptying and nutrient exposure on endothelial function.
  • To correlate glycemic responses with changes in FMD.

Main Methods:

  • A randomized crossover study involving 12 healthy subjects.
  • Three meals were tested: a standard mashed potato meal, the same meal with guar gum (slowing gastric emptying), and the standard meal consumed over 60 minutes.
  • Measurements included brachial artery FMD, blood glucose, serum insulin, and gastric emptying via breath test.

Main Results:

  • Adding guar gum slowed gastric emptying, reduced postprandial glucose and insulin, and caused a delayed but sustained FMD suppression.
  • Consuming the meal over 60 minutes resulted in smaller glycemic and FMD decrements compared to the guar gum meal.
  • A direct relationship was observed between the increase in blood glucose and the reduction in FMD.

Conclusions:

  • Postprandial endothelial function in healthy individuals is sensitive to changes in gastric emptying and meal consumption patterns.
  • Altering gastric emptying and nutrient exposure duration influences both glycemic control and endothelial function.
  • The degree of hyperglycemia correlates with the impairment of FMD post-meal.
