In the CNS, neurogenesis, the birth of new neurons from stem cells, is limited to the hippocampus in adults. In other regions of the brain and spinal cord, neurogenesis is almost non-existent due to inhibitory influences from neuroglia, especially oligodendrocytes, and the absence of growth-stimulating cues. Background:

Schwannomas are tumors arising from Schwann cells of the peripheral nervous system.
Cellular schwannoma is a histological subtype, often indistinguishable prognostically from conventional schwannomas.
Complete resection is crucial to prevent local recurrence, even in typically benign variants.

Observation:

A patient presented with a cellular schwannoma in the posterior mediastinum.
The tumor was initially resected.
A recurrence was noted 13 years after the initial surgery.

Findings:

Cellular schwannomas, despite their typical prognosis, possess the potential for late local recurrence.
The posterior mediastinum is an unusual location for schwannoma recurrence.
Long-term surveillance may be warranted for patients with completely resected cellular schwannomas.

Implications:

This case underscores the importance of thorough surgical evaluation and complete tumor removal for cellular schwannomas.
It suggests that even histologically benign-appearing tumors can exhibit aggressive behavior or late recurrence.
Extended follow-up protocols might be necessary for patients diagnosed with cellular schwannoma, particularly in challenging locations like the mediastinum.
