Background:

  • Isolated iliac artery aneurysms (IIAAs) pose a significant risk of rupture.
  • Endovascular repair has emerged as a less invasive alternative to open surgery.

Purpose of the Study:

  • To review the experience and outcomes of endovascular treatment for isolated iliac artery aneurysms (IIAAs).
  • To evaluate the effectiveness and safety of endovascular treatment in both emergency and elective settings.

Main Methods:

  • A retrospective review of 45 patients with 59 IIAAs treated between May 2005 and September 2013.
  • Evaluation of technical success, long-term patency, complications, and mortality.
  • Patients were categorized into emergency and elective treatment groups.

Main Results:

  • High technical success rate (97.8%) and excellent long-term patency (primary 95.5%, secondary 100%).
  • Complete aneurysm exclusion achieved in 84.5% of cases.
  • Low perioperative mortality (4.7%) with an overall endoleak rate of 15.5% (mostly type II).

Conclusions:

  • Endovascular treatment of iliac aneurysms (EVIAR) is an effective and preferred treatment for IIAAs.
  • This approach demonstrates favorable outcomes in both urgent and planned interventions.
  • Internal iliac artery embolization can be avoided in select cases, preserving pelvic perfusion.
