Background:

Endometriosis is a condition where tissue similar to the lining of the uterus grows outside the uterus.
Perineal endometriosis is an uncommon manifestation, often presenting as a palpable mass.

Observation:

A 28-year-old woman with no prior health issues reported a year-long history of a painful lesion in her left perineum.
Physical examination revealed a palpable mass in the affected area.

Findings:

Histopathological examination confirmed multiple foci of endometriosis.
The foci consisted of endometrial glands and stroma within dense fibro-elastic tissue.

Implications:

This case highlights the importance of considering endometriosis in the differential diagnosis of perineal masses.
Surgical excision under local anesthesia is an effective treatment for symptomatic perineal endometriosis.
