Background:

  • Dacryops are cystic dilatations of lacrimal gland ducts.
  • Caruncular dacryops arise from the lacrimal caruncle.
  • Previous reports often describe collapsed or ruptured cysts.

Observation:

  • A case of an intact caruncular dacryops in a 58-year-old male is presented.
  • The cyst exhibited cystic dilation of lacrimal gland ducts with secretory globules.
  • Histopathologic examination revealed double-layered cuboidal epithelium with goblet cells.

Findings:

  • Immunohistochemistry showed a cytokeratin (CK) profile identical to the conjunctiva.
  • The absence of myoepithelium was noted in the cystic ducts.
  • The findings support a conjunctival origin for caruncular dacryops.

Implications:

  • This is the first report of a completely excised caruncular dacryops.
  • Understanding the immunohistochemical profile aids in differentiating dacryops from other lesions.
  • This case provides valuable insights into the nature and origin of caruncular dacryops.
