Background:

Primary renal lymphoma is an uncommon kidney malignancy, accounting for less than 1% of all kidney lesions.
Early diagnosis and effective treatment are crucial for managing this rare condition.

Observation:

A 42-year-old male presented with abdominal pain and a large, 14x12x18 cm, lobulated mass involving the entire left kidney and encasing renal vessels and ureter.
Computed tomography revealed a homogeneously enhancing mass, suggestive of a significant renal abnormality.

Findings:

Histopathology confirmed diffuse large B-cell lymphoma, characterized by specific marker expression (LCA, CD20, PAX 5, Bcl 2) and a high MIB-1 labeling index (40%).
The patient received eight cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone) chemotherapy.
