Treatment for Pulmonary Arterial Hypertension: Receptor Tyrosine Kinase Inhibitors and Calcium Channel Blockers01:26

Treatment for Pulmonary Arterial Hypertension: Receptor Tyrosine Kinase Inhibitors and Calcium Channel Blockers

673
Receptor tyrosine kinase inhibitors (TKIs) and calcium channel blockers (CCBs) are two critical categories of drugs employed in the treatment of pulmonary artery hypertension (PAH). PAH is a disease that causes high blood pressure in the pulmonary arteries, resulting in chest pain, fatigue, and shortness of breath.
TKIs, such as imatinib (Gleevec), are particularly effective in tackling the growth and mitogenic factors that become upregulated in PAH patients. These factors contribute to the...

Pulmonary Tuberculosis V

947
Medical management of tuberculosis (TB) patients involves a comprehensive approach that includes diagnosis, treatment, and monitoring. The specific strategies can vary depending on the type of tuberculosis (latent or active), the patient's overall health status, and other considerations.
Latent tuberculosis infection occurs when TB bacteria are present in a person's body, but are not causing illness or symptoms. It is not contagious, and preventive treatment is crucial to avoid the...

Treatment for Pulmonary Arterial Hypertension: Prostacyclin Receptor Agonists

628
Prostacyclin receptor agonists are a class of therapeutic agents integral to managing pulmonary arterial hypertension (PAH). These drugs operate by mimicking the action of prostaglandin I2, or PGI2, a naturally occurring compound in the body.
These agonists bind to the IPR receptor situated on the plasma membrane of the pulmonary artery smooth muscle cells. This binding triggers a cascade of reactions known as the GS-AC-cAMP-PKA pathway. This pathway results in the relaxation of smooth muscle...

Combination Therapies and Personalized Medicine

6.4K
Combining two or more treatment methods increases the life span of cancer patients while reducing damage to vital organs or tissue from the overuse of a single treatment. Combination therapy also targets different cancer-inducing pathways, thus reducing the chances of developing resistance to treatment.
The combination of the drug acetazolamide and sulforaphane is a good example of combination therapy to treat cancer. The cells in the interior of a large tumor often die due to the hypoxic and...

Drug Therapy

424
The advent of drug therapy has profoundly shaped modern mental health care, providing targeted treatments for a range of psychological disorders. Psychotherapeutic drugs, classified into antianxiety, antidepressant, and antipsychotic medications, address symptoms across anxiety disorders, mood disorders, and schizophrenia. While these medications have transformed patient outcomes, they require careful management due to their potential side effects and limitations.

Transmission-based Precautions II: Airborne and Protective Environment

2.2K
Transmission-based precautions are for patients infected or suspected to be infected (or colonized) with organisms posing a significant risk to others. The transmission precautions include airborne and protective environment precautions.
Airborne precautions:
Use airborne precautions when treating patients known or suspected to have diseases that spread through the air—for example, tuberculosis or measles. These organisms are present in smaller droplets expelled by an infected person and...

Area of Science:

  • Orthopedics
  • Hematology
  • Sports Medicine

Background:

  • Climbing offers physical and mental health benefits, serving as an established therapy.
  • Its utility for people with hemophilia (PWH) remains largely unproven.
  • Pharmacokinetically (PK)-tailored prophylaxis is crucial for managing physical activity in PWH.

Observation:

  • A 25-year-old male with severe hemophilia A participated in an 8.5-month climbing program.
  • The program utilized PK-tailored prophylaxis, targeting specific factor activity levels for varying impact activities.
  • Assessments included bleeding frequency, factor consumption, joint health (HJHS), quality of life (Haemo-QoL-A), and climbing performance.

Findings:

  • Climbing therapy proved safe, with bleeding events decreasing from 14 to 1.
  • PK-tailored prophylaxis reduced factor VIII consumption compared to standard prophylaxis.
  • Significant improvements were noted in joint range of motion and swelling, with enhanced climbing ability.

Implications:

  • Combining PK-tailored prophylaxis with therapeutic climbing offers a safe and effective strategy for improving clinical outcomes in severe hemophilia.
  • This tailored approach enhances joint health, reduces bleeding, and boosts quality of life.
  • It supports increased physical activity participation for individuals with hemophilia.
