Background:

  • Diffuse large B-cell lymphoma (DLBCL) is a common non-Hodgkin lymphoma.
  • Ocular adnexal lymphoma (OAL) involves the structures around the eye.
  • The clinical features of ocular adnexal DLBCL (OA-DLBCL) have not been extensively studied in large cohorts.

Purpose of the Study:

  • To investigate the clinical features and outcomes of patients with OA-DLBCL.
  • To identify prognostic factors for survival in OA-DLBCL.

Main Methods:

  • Retrospective international cooperative study involving 6 eye cancer centers.
  • Analysis of 100 patients with OA-DLBCL over a 30-year period with a median follow-up of 52 months.
  • Evaluation of overall survival, disease-specific survival (DSS), and progression-free survival.

Main Results:

  • The cohort included 100 patients (median age, 70 years; 54% female).
  • Primary OA-DLBCL comprised 57% of cases, with most presenting at Ann Arbor stage IE and TNM T2.
  • The 5-year overall survival was 36.0% (median 3.5 years). Increasing TNM T category predicted decreased DSS in primary OA-DLBCL.
  • Female sex and Ann Arbor stage predicted DSS in the overall cohort.

Conclusions:

  • Primary OA-DLBCL often presents with early-stage disease (Ann Arbor IE, TNM T2).
  • The 5-year survival for OA-DLBCL is approximately 36%, with a median survival of 3.5 years.
  • TNM staging, Ann Arbor stage, and patient sex are important prognostic factors for DSS in OA-DLBCL.
