Advanced nursing practice in Kenya shows nurses deliver expert care, but lack the autonomy typical of advanced roles. Educational pathways and regulatory frameworks hinder the development of advanced nurse practitioners in sub-Saharan Africa.

Background:

Limited literature exists on advanced nursing practice in sub-Saharan Africa.
This study investigates the presence and potential for advanced nursing practice in Kenya.

Purpose of the Study:

To explore the current state and future possibilities of advanced nursing practice within Kenya.
To understand the roles and autonomy of experienced nurses in diverse healthcare settings.

Main Methods:

Qualitative study employing semi-structured interviews with ten purposively selected nurses.
Interviews covered urban, rural, community, hospital, public, and private healthcare settings.
Thematic analysis was used to interpret transcribed interview narratives.

Main Results:

All interviewed nurses provided expert, evidence-based care.
Most nurses engaged in administrative duties, teaching, and advocacy.
Only nurses in private practice demonstrated autonomy comparable to advanced nurse practitioners.

Conclusions:

Advanced nursing practice, as defined internationally, was not identified in Kenya.
Nurses with lower qualifications exhibited greater autonomy than highly qualified ones, who often moved into management or education.
Lack of regulation for advanced practice roles and the presence of 'clinical officers' may impede the development of advanced nurse practitioners, despite a clear need for their expertise in achieving health goals.
