Background:

Vascular surgeons frequently encounter patients seeking second opinions for venous ablation therapy.
Many self-referred patients present with conditions not requiring intervention or are recommended excessive procedures.

Purpose of the Study:

To highlight the issue of overtreatment in venous ablation.
To explore potential avenues for addressing and rectifying substandard medical practices in venous disease management.

Main Methods:

Case review of patients seeking second opinions for venous ablation.
Consultation with peers from the American Venous Forum regarding ethical dilemmas.
Reporting egregious cases to the state medical board.

Main Results:

A significant number of patients were found to have no venous reflux, rendering ablation unnecessary.
Some patients were recommended an excessive number of ablations (3-7 per leg) when only 1-2 were indicated.
Previous reports to the state medical board yielded no action.
