Summary

Infraslow EEG activity (ISA) was present in most infantile spasms (IS) and may indicate a positive treatment response. Exclusive generalized ISA patterns suggest a better prognosis for infantile spasms.

Background:

  • Infantile spasms (IS) represent a severe epilepsy syndrome in infants.
  • Infraslow EEG activity (ISA) analysis is valuable for epilepsy evaluation.
  • ISA's role in IS pathophysiology remains unexplored.

Purpose of the Study:

  • To characterize ictal ISA patterns in infantile spasms.
  • To correlate ISA patterns with clinical data and treatment outcomes in IS patients.

Main Methods:

  • Review of video EEG recordings for IS cases over 10 years.
  • Analysis of ISA (generalized, lateralized, or absent) during spasms.
  • Correlation of ISA findings with clinical course, treatments, and etiology.

Main Results:

  • 77% of spasms showed associated ISA (57% generalized, 20% lateralized, 21% absent).
  • Exclusive generalized ISA correlated with better initial therapy response (100%) compared to lateralized (66.7%) or absent (50%).
  • Generalized ISA was associated with fewer co-occurring seizure types.

Conclusions:

  • Ictal ISA is a common finding in infantile spasms.
  • Exclusive generalized ISA patterns may predict a favorable prognosis in IS.
