Summary

A rare melanotic neuroectodermal tumor was found in the pineal region of a child. This malignant tumor showed epithelial and neuroblastomatous features with melanin production, likely originating from the fetal pineal gland.

Background:

  • Melanotic neuroectodermal tumors (MNTs) are rare neoplasms of neural crest origin.

  • Understanding the origin and behavior of rare pediatric brain tumors is crucial for diagnosis and treatment.

Observation:

  • A case of a 4-year-old girl with a diffusely meningeal-spreading melanotic neuroectodermal tumor of the pineal region is presented.
  • Histological examination revealed predominantly epithelial elements with tubules, cords, and nests, alongside a neuroblastomatous focus.
  • Melanin pigment was abundant in tumor cells and macrophages; no teratoid elements were identified.

Findings:

  • Immunohistochemistry showed tumor cells positive for neuron-specific enolase but negative for S-100 protein, epithelial membrane antigen, glial fibrillary acidic protein, vimentin, alpha-fetoprotein, and human chorionic gonadotrophin.
