Summary

Extra-axial ependymomas are rare tumors outside the brain. This case details an intraperitoneal ependymoma recurring over 24 years, highlighting its unusual presentation and behavior.

  • Extra-axial ependymomas are rare tumors originating outside the central nervous system.
  • Most extra-axial ependymomas exhibit benign clinical behavior.

Observation:

  • An elderly woman presented with an intraperitoneal ependymoma.
  • The tumor recurred twice over a 24-year period following initial surgical excision.
  • Histological examination revealed consistent features, including rosettes and perivascular pseudorosettes, at each recurrence.

Findings:

  • Immunohistochemistry demonstrated strong positivity for glial fibrillary acidic protein, estrogen receptors, CAM 5.2, and CD99.
  • Staining for pankeratin was weak, while epithelial membrane antigen (EMA) and S-100 were negative.
  • Ultrastructural analysis confirmed neuroectodermal features, such as filamentous cytoplasm and intercellular bridges.
