Summary

Robot-assisted radical cystectomy using the da Vinci Surgical System is a safe and effective procedure for bladder cancer. This study shows it achieves satisfying oncological and functional outcomes, including continence and erectile function, within acceptable timeframes.

Background:

  • Robot-assisted laparoscopic cystectomy has emerged as a technically feasible surgical option.
  • Data on the technical and functional outcomes of this procedure in large patient series are limited.

Purpose of the Study:

  • To evaluate the technical and functional results of robot-assisted laparoscopic radical cystectomy using the da Vinci Surgical System (DVSS).

Main Methods:

  • A series of 27 patients underwent DVSS radical cystectomy between 2004 and 2005 for muscle-invasive bladder cancer or radiation-induced bladder shrinkage.
  • Procedures included pelvic lymphadenectomy, with urinary diversion via ileal conduit or ileal neobladder.
  • Follow-up data were collected to assess complications, oncological outcomes, and functional results (continence, erectile function).

Main Results:

  • Mean operating time was 340 minutes with a mean blood loss of 301 ml.
  • Negative surgical margins were achieved in all but one case.
  • Short-term follow-up revealed low complication rates and promising functional outcomes, including satisfactory erectile function in 6/7 patients and complete daytime continence in 7/8 patients.

Conclusions:

  • Robot-assisted laparoscopic cystectomy with DVSS is a safe procedure for bladder cancer treatment.
  • Satisfactory short-term oncological and functional results, including urinary continence and erectile function, can be achieved.
  • The procedure demonstrates acceptable operating times and complication rates.
